If we don't have your insurance in time, we can't verify your benefits before you arrive — and that usually means we can't do treatment the same day. Grab your card and fill this in now while you have a second.
No insurance? You're all set — ask us about the Elite Gold Plan when you're in. Card not handy? We'll text you a secure link as a backup.
